Support Broker for Medicaid Waiver Consumer Directed Options Program

Seeking applicant for a full time position.  Responsibilities include performing in-home assessments and case management/support brokerage services for Medicaid programs that provide services for the aging and disabled populations.  The preferred candidate will have a Bachelors or Masters Degree in Social Work, Psychology or Bachelors or Masters Degree in Nursing with current Kentucky nursing license.  Ability to speak Spanish beneficial.  Reliable transportation required.
